When do we have to do surveys?

  • Baseline – Parent & Child
  • 3-Month – Parent & Child
  • 6-Month – Parent & Child
  • 12-Month – Parent & Child
  • 18-Month – Parent & Child

Where can I find my survey?

The surveys are emailed to you at the start of each time point.

  • Check your email for the subject lines:
    • The SUPPER Project: Time for X-Month Follow Up Assessment and Link for Adult Survey
    • The SUPPER Project: X-Month Follow Up Child Survey
  • There will be a separate email for your survey and your child’s survey
  • You can use this link in the email to return to the survey at any point and it will bring you to wherever you left off. This is helpful if you want to complete the survey in chunks!

Survey not loading or you’ve hit a blank page on the survey?

Sometimes the survey is a bit glitchy—just keep clicking to the next page until it brings you there. It might take a couple of clicks!

Some questions don’t apply to my child.

All of the survey questions can be answered by your child regardless of your child’s own experience or knowledge.

  • Some of the questions are hypothetical and ask your child to imagine how doing an activity might make a person feel – some of these questions are phrased like “when I do this, I feel this way”.
  • Even though the questions are phrased like that, they are still hypothetical, and your child should still answer them based on how your child thinks the activity would make a person feel. Remind your child of the skydiving example we went over during the first visit.
